A Brazilian Butt Lift (also known as BBL Surgery or Buttocks Augmentation) is a surgical procedure indicated for modifying the contour of the buttocks through harvesting enough fat from one or more areas and transferring back to the buttocks and hips. How is a BBL done?
A BBL works by removing excess fat using liposuction mainly from the trunk. The fat is transferred, or grafted, into the upper and outer buttocks to make it seem as if it has been lifted. Some patients have excess fat in the arms or thighs, and these areas can also be treated with liposuction to add more volume.
A BBL procedure is best performed under general anesthesia provided by an anesthetist, at minimum. Be extremely careful if they offer you a BBL under local anesthetic. The trunk is a significantly large area of the body that would require a dangerous amount of local anesthetic to numb in order to perform a BBL.

Who is a good candidate for BBL surgery? How much should I weigh to get a BBL?
The ideal candidate for a BBL procedure is a healthy patient who is at a near optimal weight. In our opinion the best results are seen when the patients have a BMI of 23 – 25. At Cocco Clinique MD, we encourage patients to come as close to a BMI of 25 as possible. Also, for safety reasons, we rarely perform a BBL in patients with BMI above 30.
What additional procedures are involved during a BBL?
The most performed adjunct procedures during a BBL are a mommy makeover, which can add Tummy Tuck, breast augmentation and/or breast lifting, or genital surgery such as labiaplasty and vaginal tightening. Nevertheless, in theory, any procedure that takes less than 4 hours could be incorporated into the usual 2-hour BBL to remain within our safety threshold of 6 hours of operation.
BBL vs Butt implants – Which one is better?
Fat Grafting to the buttocks brings more subtle results than butt implants. However, it has the advantage that is potentially longer lasting without distortion of the buttocks long term. Butt implants have greater visibility so they are a good option for patients who lack significant volume. Butt implants have the disadvantage of increased mobility as the patients sit and may shift the implants with time. Also the buttock skin may sag significantly when implants are removed creating nearly-impossible to correct deformities. The down side of BBL may be palpable lumps from fat that is transferred, and that did not survive and created scarring. The vast majority of time, these palpable deformities are not visible nor felt when sitting or laying, and thus can be ignored. There are cases when both butt implants and fat grafting to the buttocks can be beneficial to the desired contour and performed simultaneously.
Is BBL surgery dangerous?
A few years ago, BBL surgery had a bad reputation for being dangerous, but this was before research showed the correct injection techniques which directed surgeons to avoid injecting directly into the muscle layers. BBL surgery is currently a safe procedure when both the patient and the surgeon have been vetted. The patient should be medically evaluated and determined to be a good candidate for BBL in Dallas. It is imperative that the patient is transparent about disclosing previous medical history as well as lifestyle habits. The surgeon’s qualifications and physician reports to the Texas Medical Board should be researched thoroughly.
Why are BBLs so cheap in Miami? Is getting a cheap BBL near me a good idea?
The only way to offer cheap surgery is to perform several procedures per day (high volume of patients) and have minimal follow up encounters on these cases, such as in medical tourism where patients travel back to their homes far away shortly after surgery. The state of Florida put a limit as to how many BBL’s can be done by one surgeon per day to keep surgeons from booking several cases at a time.
Unfortunately, some doctors were caught using assistants who were foreign doctors without a license to help with the volume. This turned out to be a deadly decision and the state of Florida was dealing with an extremely high death rate. It simply is not feasible to do multiple cases per day by the same surgeon. Liposuction is physically demanding, a heavy upper body workout. Ask your surgeon or consultant how many BBL cases your surgeon will perform the day of your surgery? This will give you a clue.
Should I gain weight to prepare for a BBL?
You should absolutely NOT gain weight for a BBL! If you’re wondering – why is my abdomen so big after my surgery? Here is why: When we gain weight, we cannot control where the excess fat cells accumulate. Unfortunately, all this fat will go to surround the internal organs of the trunk internally. We call this visceral or central fat and it cannot be retrieved with liposuction. So if you gain weight before surgery, we will not be able to deliver the smallest possible. Unfortunately this is a common misconception and a bad decision.
Can I get a BBL if I’m underweight?
Can I get a BBL if I am 100 pounds? Most of the time, the answer is: yes. We can find excess fat deposits in select areas in the thinnest of patients.
Brazilian Butt Lift Recovery Time and Medications
The recovery time is generally broken into stages. The first 24 to 48 hours after surgery we require having an adult companion constantly to supervise activities of daily living.
- For the first week or two and as pain and mobility improve, you will still need help by having someone oversee household activities and meals.
- By week three most people are able to return to work, do housework and housekeeping.
- We encourage abstaining from driving for about 2-3 weeks, and most definitely when taking prescribed pain medication.
- No strenuous exercise, heavy lifting or straining for the first 4 weeks.
- No long-standing pressure on the buttocks or gluteal exercise for at least 6-8 weeks.
Prescribed medications after this procedure include an antibiotic, pain medication and an over the counter anti-inflammatory. The antibiotic should be taken until finished. The pain medication should be weaned as soon as possible and switch to over-the-counter medications such as Acetaminophen and Ibuprofen.
Is BBL the best treatment for loose skin?
Yes and no. While the loose skin on the buttocks will be filled out, the skin on areas of liposuction where fat is harvested will be more loose. So BBL surgery is not recommended when there is excessive laxity of the abdominal skin. In these cases, a Tummy Tuck would be more beneficial. Also, in cases where the thighs are treated with liposuction, this could result in more cellulite.
When will I see cosmetic results from a BBL?
We counsel our patients that they will see the earliest results around 6 months after a BBL surgery. This is because it takes a while to tone and recondition after the initial recovery period which lacks all strenuous activities including toning exercise.
How can I maintain the cosmetic results of my BBL?
The best way to maintain the cosmetic results of a BBL is to maintain a healthy lifestyle, healthy diet, and a regular fitness routine that includes cardio and weightlifting toning exercises. You should avoid dramatic weight fluctuations, and focus on maintaining your weight below the target-weight given to you during your postoperative appointments.

A BBL can change the way clothing fits, thus improving self-esteem. You have to be very specific with your request, because some patients get a BBL to rejuvenate the buttocks, whereas others simply want a significant volume increase. It is not the same technique at all. For patients who want a significant size change, we recommend staging BBL’s because over-injecting can be detrimental to the fat survival, and it can even be dangerous with a higher likelihood of a fat embolism or, if it creates too much pressure on the nerves that travel to the legs.
Who should perform a BBL procedure?
BBL surgery should be performed by a Board-certified Plastic Surgeon near you. Specifically, those board-certified by the American Board of Plastic Surgery. We place emphasis on the words Plastic Surgeon because they are different from Cosmetic Surgeons. Legally, any medical doctor with an MD or DO degree could perform a BBL as a cosmetic procedure, but do you want your surgeon to meet the minimum required to do a BBL? Or, do you want the most qualified surgeon to do it? Plastic surgeons were medical students who had to be ranked at the top 10 percentile of U.S. medical schools to get into an American PLASTIC SURGERY residency. Then, residency lasted anywhere from 6-8 years where they took care of the most complex of reconstructions, to rotating with some of the best cosmetic surgeons in the country.

No, we do drainless BBLs. Dr. Cocco does not use drains 99% of the time and if she ever did, they would exit out of the incision itself, not from intact skin areas such as the flanks or pubis which could be potentially more painful or leave additional scars.
Where does all the fluid go during a drainless BBL? The key is in the postoperative massage which we will teach the patient the day after surgery. We also use specialized compression garments, or fajas, to significantly reduce friction which results in decreased serous fluid production. So, no need for drains after BBL!
Why is it called a Brazilian Butt Lift?
The procedure originated in Brazil where it was popularized long before the trend migrated north. The way the fat is first harvested from the waist and then injected into the buttocks to contour both waist and buttocks; make it seem as though the buttocks have been “lifted,” though an actual butt lift is not performed.
Will I lose my BBL if I work out or lose weight?
No, most likely you will lose weight proportionally and, we believe the resistant fat that was located in the flanks will stay resistant to weight loss when transferred to your buttocks.
Brazilian Butt Lift gone wrong?
We feel BBL surgery is an artform and the buttocks should be contoured in a way that looks pleasant and proportional to the patient’s thighs and trunk. The hips and buttocks should be increased in size but shaped so that they appear rejuvenated and not sagging or grotesque. A BBL can really go wrong, when overzealous techniques are used injecting into the gluteal muscles. This can be dangerous and the reason Brazilian butt lift death rates were so high just a few years ago. Most BBL deaths were due to fat being injected into large veins in the deep buttocks and this fat traveled to the heart, lungs and brain (fat embolism).
How painful is a BBL?
You will not be experiencing pain during the BBL procedure itself because you will be under general anesthesia. However, BBL surgery is expected to be moderately painful with the pain starting a few hours after surgery. The most intense pain is normally experienced the first two days, and this is controlled with prescription pain medication, which is weaned over the next week. Swelling and bruising are common in the first 2 weeks after surgery. By week two most patients are able to tolerate pain with over the counter medications. Although skin sensation will be disrupted for a few weeks, pain lasting beyond this is extremely rare and treated with desensitization massage.
When should BBL surgery be performed?
Patients who are considering having a BBL should be at, or near, their ideal body weight and such weight remained stable for at least 3-6 months.
How soon after pregnancy can I do a BBL?
We recommend waiting a minimum of 1 year after the last pregnancy to consider having a BBL. If you’re having or just had a baby, we recommend breastfeeding and begin exercising as soon as the obstetrician approves in order to lose weight closest to ideal body weight before seeking a consultation for a BBL with a plastic surgeon.
Do I have to wait until I’ve finished having children to have a BBL?
No, a BBL can be done before having any children. There are young patients with disproportionate body measurements who would benefit from having a BBL.
Can you take all my body fat at once during a BBL?
We do not recommend taking all the fat possible during a BBL and, for one, it may not ALL fit into the buttocks. Also, it is important to be aware of the limitations when getting a Brazilian Butt Lift in Dallas, TX. For safety reasons, the higher the BMI the lower the chance of being able to remove all the fat since liposuction limit in the state of Texas is 5 liters.
